Chin Reduction or Augmentation
Chin projection should be balanced with the upper face.
A small chin can be augmented by advancing the lower part of the chin or by simply inserting a Medpor implant which is fixed to the bone with a small titanium screw. It is unusual to perform a chin augmentation in FFS patients.
Most often the chin needs to be reduced by either a set back or by simply burring the sides to reduce the squareness of the chin.
A preliminary cephalometry is useful in planning a setback.
Chin surgery is done with an incision inside the mouth.
This is done with an incision inside the mouth. On rare occasions, a too prominent chin may be reduced by resetting it in a more posterior fashion.
Setting back the chin must be done carefully since it can worsen a loose neck. Risks are very low and include nerve injury with partial or total loss of lip sensation.
